Position Objective Provide leadership and oversight of the community and/or territory to ensure the highest level of care and customer service standards are upheld throughout the IHPS Team within designated community and/or territory.
Establish and maintain professional relationships internally and externally.
Ensure IHPS standards are met, and all provided services are in compliance with applicable federal, state and local laws and regulations.
Responsibilities - Ensures compliance with all applicable federal, state, and local laws and regulations.
- Maintain a working knowledge of all IHPS policies and procedures and ensure compliance.
- Offer new ideas and suggestions for new policies and procedures both community and companywide.
- Ensure all Illinois Department of Public Health regulations outlined in the Administrative Code pertaining to Home Health, Home Services, and Home Nursing Agency are upheld.
- Establish and maintain a strong relationship with clients, families, and referral sources.
- Respond promptly to all client inquiries and address any concerns.
- Accommodate assessments, start of service procedures, and timely initiation of care.
- Oversee the endorsement of caregivers to new cases, effectively communicating client care needs and family expectations and observing the caregiver's abilities to appropriately accommodate the level of care.
- Provide ongoing quality assurance visits to each individual case, continuously assessing clients’ needs and caregiver performance.
- Resolve client concerns promptly and appropriately and report resolution to Supervisor.
- Schedule training and host orientation to new team members, certifying competency of quality-of-care standards, policies and procedures before graduating team members to work independently.
- Responsible for creating schedules and assignments, and overseeing those assignments are staffed to meet the care needs of the client while effectively managing payroll.
- Oversee the performance of assigned team members, formally and informally provide coaching, intervention, feedback and recognition.
- Submit accurate and complete documentation in a timely manner, in accordance with IHPS policies and procedures.
- Ensures that all team member’s documentation is accurate, complete and appropriate.
- Oversees compliance with risk management program, reviewing all occurrences and formulating appropriate corrective measures and follow up protocol.
- Perform scheduled assignments and provide hands on care to clients and cover assignments when necessary.
- Ensures quality control, effective waste and supply management, and all IHPS property is in good and working order, communicating any needs to the appropriate department and ensure that needs are met.
- Participates in all mandatory in-services, meetings, and quality reviews in accordance with company policy and/or state regulations.
- Be available during all hours of operation for IHPS.
- Performs all other duties as determined by the Regional Director of Services.
Qualifications and Experience - Possesses strong interpersonal, written and oral communication skills.
- Excels in a fast-paced environment.
- Organized and detail oriented.
- Manages time and tasks effectively.
- Demonstrates strong customer service and problem-solving skills.
- Computer literacy on basic MS WORD, MS EXCEL, and MS OUTLOOK.
- At least one (1) year of experience in home services or health care, preferred.
- A team player with the confidence to take the lead and guide other team members.
- Exudes a passion and desire to care for the elderly.
Position Requirements - Must pass a criminal background check in accordance with the Health Care Worker Background Check Act (225 ILCS 46) and Health Care Worker Background Check Code (77 Illinois Administration Code 955).
- Must submit to and have negative results in a drug and alcohol screening.
- Provide proof of a recent TB test with negative results.
- Provide a copy of a health exam by a licensed physician no less than 12 months prior to the date of hire.
- Copy of certification/ licensure if applicable.
- Must have a valid driver’s license or reliable transportation for field work.
- Must be eligible for employment in the United States.
Physical Demands Physical demands include stooping, bending, turning, stretching, lifting to assist clients, moving equipment, etc.
visual and auditory acuity necessary to detect changes in client condition.
Manual dexterity required to handle equipment and assistive devices.
Job Type: Full-time Pay: $38,000.
00 - $41,000.
